2009 ISQED Registration Information
Please note
carefully that there are two methods for registration. The first method is by
on-line registration which is the preferred method since it is fast and
efficient. The second method, the
Mail-In or FAX method, is more appropriate if you are planning to pay using check
(but can also be used when you pay by
credit card or company purchase order). In this method, you
should print and fill out the registration
form and send it along with the check to the address provided on the form.
If you are paying by credit card make sure to provide the name of the card
holder as appears on the card and the accurate billing address. Also to ensure
your registration you need to make sure your credit card
number and expiration date is entered correctly otherwise you might not
qualified for the advance registration rate. Please do not hesitate to email

ON-LINE
SECURE REGISTRATION (when paying with Credit Card). Please note the on-line
registration uses 256-bit encryption for enhanced security. Registrants outside
of USA might have problem accessing it if you do not have a web browser supports
256 bit encryption. In that case you can (see following).
2. MAIL-IN or
FAX (when paying with US bank Check or Credit Card)

registration form, fill all the fields, include your credit card information,

Payment of the Technical Session registration fee entitles the registrant to
one copy of the conference proceedings , Monday evening Panel, Wednesday lunch
Panel, and entrance to all technical sessions. Technical Session registration
alone does not include entrance to the tutorials
and workshops.

CANCELLATION POLICY:
Requests for cancellation or refund must be received
before March 5th, 2009. A $50.00 processing fee will be
withheld from all refunds.
